WIC Program Overview

The Women, Infant, and Children (WIC) is a special supplemental Nutrition Program that is designed to provide healthy foods, breastfeeding support, nutritional education and other social services referrals to women who are pregnant, breastfeeding, or postpartum. Infants and children up to their 5th birthday are able to participate in this program as well.

  2. Breastfeeding women up to their infants first birthday.
  3. Post-partum women
  4. Infants
  5. Children up their 5th birthday.
  1. Live in the state of Louisiana and
  2. Meet the income guidelines or be enrolled in Medicaid or receive SNAP or TANF Benefits.
  1. Breast pumps and supplies for breastfeeding moms per WIC guidelines
  2. Breastfeeding peer counseling to help with breastfeeding concerns.
  3. Healthy food packets which may include fresh fruits, vegetables, whole grain bread, brown rice, canned tuna or salmon, eggs, cheese, low fat mild, iron-fortified cereal, dry beans, peanut butter and fruit juice.
  4. Iron-fortified infant formula, iron-fortified infant cereal and single ingredient infant fruits and vegetables.
  5. Infant meats for fully breastfed infants.
  6. Nutritional classes and infant/child feeding tips.
  7. Referrals to health and social services.
  1. To be eligible, applicants must have a total household income at or below a level set by the USDA.
  2. Current income guidelines can be viewed at www.wic.dhh.louisiana.gov
  3. Income eligibility is determined by gross income, and some exclusions may apply.
  4. One pregnant woman counts as two household members for income eligibility.

Each applicant for WIC should provide documented proof of identification, residency, household income, and pregnancy (when applicable).

Please see below for a detailed list of what you need:

  1. Each person applying for WIC should come to the clinic on you appointment (or welk in) date. Please bring an extra diaper for infants and children who are applying for WIC since they will be weighted with a clean, dry diaper.
  2. Immunization Records: please bring immunization records for all infants and children applying for WIC, if available.
  3. Proof of identification: You need to bring identification for yourself and for each person who is applying for WIC. The following are accepted proof of identification:
    1. For Infants and children:
      1. Birth certificate of Birth Record
      2. Nursery ID Bracelet
      3. Valid Medicaid Card
      4. Medical Record, insurance Card, or Immunization records.
    2. For Adults:
      1. Birth Certificate or Medical Records (excluding immunization record).
      2. Current Driver’s License or State Issued ID Card
      3. Current Passport or Immigration Card
      4. Valid Insurance Card or Medicaid Card
      5. Valid Voter Registration Card
  4. Proof of Residency:
    1. For infants and children, proof must be provided for the parent/care taker also:
    2. You need to bring proof of where you live.
    3. These documents should include you name, physical address, city, state, and zip code. PO Boxes are not accepted.
    4. The following are acceptable for proof of residency:
      1. Valid Medicaid Card
      2. Valid Voter Registration Card
      3. Current Rent of Mortgage receipt showing address of residence and name of renter.
      4. Current pay stub showing employee address.
      5. Current utility bill (gas, electric, water, phone, etc).
      6. State or local document indicating Louisiana residence, i.e property tax notice.
      7. Processed mail addressed to the participant or parent/caretaker at the physical address.
      8. For migrants of victims of theft, loss or disaster with no proof of residence, a statement written and signed by staff of a social service agency, church, legal aid society, school counselor, or nurse attesting to residency.
  5. Proof of Income:
    1. Income for all members of the economic unit/household must be provided.
    2. If you receive Medicaid, Food Stamps, of TANF, bring one of the following:
      1. Medicaid Letter showing current eligibility of valid Medicaid Card.
      2. SNAP (Food Stamp Program) document showing current eligibility.
      3. TANF (Temporary Aid to Needy Families) document showing current eligibility.
    3. If you Do Not receive Medicaid, Food Stamps, TANF, the following are accepted:
      1. Public assistance or welfare payments for the most recent 30 days.
      2. Child support or alimony payments for the most recent 30 days.
      3. Current pay stubs or alimony payments for the most recent 30 days.
      4. Current pay stubs showing gross pay and time frame (weekly, monthly, etc) for the most recent 30 days.
      5. Current Leave and Earnings Statement (LES) for military personnel for the most recent 30 days.
      6. Social Security award letter or notice stating current earnings.
      7. Unemployment earning statement for the most recent 30 days.
      8. Retirement, pensions, or annuities payments.
      9. Scholarship letter showing the date and amount awarded.
      10. W-2 forms or income tax return for the most recent calendar year.
      11. Tax Returns for the self employed from the most recent calendar year.
      12. Signed statement from employer indicating gross earnings and time frame (weekly, monthly, etc) for the most recent 30 days.
      13. Winnings or payments from gaming, lottery, and bingo.
      14. Other cash income, including regular contributions from persons not living in the household.
